They treat both insured and uninsured patients. If you are uninsured and have a low income, you may qualify for a sliding fee for your medical, dental and pharmaceutical needs. This program is intended to provide medical care for people who otherwise may be unable to see a doctor. You are expected to pay the discounted rate or minimum fee for services. If you cannot pay at the time of service, you can make arrangements for payments. Uninsured quilify students from certain elementary schools qualify for free services to students such as: screenings and assessments to follow up care, mental and behavioral health services, nutrition and health counseling, access to a regular primary care physician, emergency first aid/CPR, chronic disease management and primary health services. Payment is required unless you qualify for discounted or free services. You can pay using commercial and private insurance, Medicare, Medicaid, Helathy Kids, and self pay.

What to bring

To qualify for uninsured discounts you must provide proof of house hold income using one of the following documents: W-2/income tax/1099, Most recent 3 check stub, Social Security letteror check stub, Notarized letter from employer if self employed, paid in cash, etc, Letter from unemployment office or unemployement check stub, Notarized Self, Declaration of Income (to be used only if the applicant does not have a written income verifcation)(Refer to Self-Declaration Form)
